THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our duty to protect health information
Any Lab Test Now® of Ashburn, VA (“we” or “us”) is required by law to maintain the privacy of your protected health information (“PHI”), provide you with this Notice of our legal duties and privacy practices, notify you following a breach of unsecured PHI, and follow the terms of the Notice currently in effect.
Uses and disclosures that do not require your authorization
We may use or disclose your PHI without your authorization for the following purposes:
- Treatment. To collect your sample, process your test, and deliver your results — including coordination with reference laboratories that perform the analysis.
- Payment. To process payment from you or, when you direct us, a third party (such as an employer paying for an employment-related test).
- Healthcare operations. To run our business, including quality assessment, training, compliance, and reviewing reference-lab performance.
- As required by law. When federal, state, or local law mandates disclosure.
- Public health activities. To public health authorities for reportable conditions, vital statistics, or product safety.
- Health oversight activities. To agencies authorized by law to conduct audits, investigations, or licensure activities.
- Judicial and administrative proceedings. In response to a valid court order, subpoena, or discovery request.
- Law enforcement. Where required by law or in response to a valid legal request.
- Coroners, medical examiners, and funeral directors. To carry out their duties.
- Organ donation. If you are a donor.
- Research. Under specific safeguards approved by an Institutional Review Board, when applicable.
- Workers’ compensation. As authorized by Virginia workers’ compensation laws.
- Serious threat to health or safety. When necessary to prevent or lessen a serious and imminent threat.
- Military, national security, or correctional facility. Where applicable.
Uses and disclosures that require your authorization
- Most uses and disclosures of psychotherapy notes.
- Uses and disclosures of PHI for marketing.
- Sale of PHI.
- Any other use or disclosure not described in this Notice.
You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
Your rights regarding your PHI
- Right to inspect and copy. You may request to inspect or receive a copy of your PHI. We may charge a reasonable, cost-based fee.
- Right to amend. You may request that we amend information you believe is incorrect or incomplete. We are not required to agree to your request.
- Right to an accounting of disclosures. You may request a list of disclosures we have made of your PHI for certain purposes.
- Right to request restrictions. You may ask us to restrict how we use or disclose your PHI for treatment, payment, or operations. We are not required to agree, except when the disclosure is to a health plan for a service you have paid for in full out of pocket.
- Right to confidential communications. You may ask us to communicate with you in a specific way (for example, by phone instead of email) or at a specific location.
- Right to a paper copy of this Notice. You may request a paper copy at any time, even if you have agreed to receive it electronically.
- Right to be notified of a breach. We will notify you in the event of a breach of unsecured PHI affecting your information.
Reference labs and the patient portal
We use accredited reference laboratories to process your samples. PHI is shared with them only to the extent necessary to perform testing, and we have business associate agreements in place with each laboratory.
Your test results are delivered to you through our secure patient portal at https://anylabportal.careevolve.com. The portal is password-protected and accessible only by you. Results are not sent to your health insurance carrier unless you specifically direct us to do so.
How to file a complaint
If you believe your privacy rights have been violated, you may file a complaint with us at the address below or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Changes to this Notice
We reserve the right to change this Notice and the changes will be effective for all PHI we maintain. The current Notice will be posted on this page with a revised effective date.
